Applicant needs to be in Singapore to sign the Deed Poll personally in our office, and wet ink is required. No virtual appointment is permitted.

If you are not a Singapore Citizen or Singapore PR, please do not use this service unless your National Immigration Authority accept a Singapore executed Deed Poll.
For Singapore PR, you will only be able to change your name in your Singapore Blue NRIC but not your name in your Foreign Passport. You are strongly advised to check with your National Immigration Authority on issues relating to your Foreign Passport and validity of a Singapore Deed Poll. ICA may require you to change your name in your Foreign Passport before you can change the name in your Singapore NRIC.
For Non-Singapore PR (Foreigner) who wishes to use our Deed Poll services, we will assist to prepare the Deed Poll, but there is no warranty that the Deed Poll will be suitable for whatever purpose(s) that you have intended for.
You should also enquire with ICA at www.ica.gov.sg on whether your proposed name change will affect your Entry or Re-Entry Permit.

For Minor's change of name (including but not limited to surname), occasionally, ICA may not process the change of name or require both parents to sign the Deed Poll, which are circumstances beyond our control. Please see ICA Change of Particulars and Single Parent's Declaration Form.

Name exactly as appearing in your ID, including any Hanyu Pinyin Name (in brackets). Eg. Tan Wei Loh (Chen Weiluo)
New Name to appear in your new IC. If deleting Hanyu Pinyin name (in brackets), please omit it. Hanyu Pinyin name (in bracket) must tally with Chinese characters. Eg. If existing name is "TAN WEI LOH (CHEN WEILOU)", and you are deleting Hanyu Pinyin name in bracket, simply state your new name as "DAVID TAN WEI LOH" without your Hanyu Pinyin name.
Applicable for Applicant who wishes to retain, add, change or delete name in Chinese, Tamil, Arabic or other Non-English Character(s). Eg. If you do not have any name in Non-English Characters (eg. name in Chinese characters), and you are not adding any name in Non-English Characters, then select option "No Non-English Characters".
