2. applying for certified true copy (CTC) of your lost Deed Poll (provided it is still in Loh Eben Ong's archive). 

COST OF CTC

1. Search Fee (depending on when Deed Poll was dated) - from $0 to $200
2. Fee for 1st CTC Cost by Advocate & Solicitor (Lawyer) and/or Notary Public
3. Subsequent CTC (within same application) - $10 each

This Form is NOT for new application for Deed Poll. Please use our Deed Poll Online for new Deed Poll. We do not warrant that your Deed Poll is in our archive even if you have done your Deed Poll with us.

For CTC of your Deed Poll (which is not lost), please use our CTC by Lawyer or Notary Public services.
